- Name / नाम : Enter your full name as mentioned on the Aadhaar Card. (“आधार कार्ड में बताए अनुसार अपना पूरा नाम दर्ज करें।”)
- Mobile No / मोबाइल नंबर : Fill the correct Mobile Number. Confirmation code will be sent on this number.“सही मोबाइल नंबर भरें। इस नंबर पर कन्फर्मेशन कोड भेजा जाएगा। “
- Email ID / ईमेल : Fill the correct Email ID. “यहां अपनी सही ईमेल आईडी का उल्लेख करें।”
- PAN Card Number / पैन कार्ड संख्या : Fill ten-digit unique alphanumeric number issued by the Income Tax Department. “आयकर विभाग द्वारा जारी दस अंकों का यूनिक अल्फ़ान्यूमेरिक नंबर भरें।”
- Date of Birth / जन्म की तारीख : Enter your date of birth as printed on Aadhaar Card. “आधार कार्ड पर छपी अपनी जन्मतिथि दर्ज करें।”
- Aadhaar Number / आधार संख्या : Enter your correct 12-digit Aadhaar number issued by the UIDAI. “यूआईडीएआई द्वारा जारी अपना सही 12 अंकों का आधार नंबर दर्ज करें |”
